How to Record Continuing Pharmacy Education Credit
In order to recieve continuing education credits at Seminar 2009, all attendees must record the CE courses they attend on a Statement of Continuing Education Form. These forms will be distributed to all attendees on-site prior to the start of Seminar 2009.
In order to validate your statement of credit, you must have it signed off by a member of CSHP staff. You may do this by returning your completed form to the Seminar Registration Desk or Information Center. CSHP will keep one copy and give you another which must be retained as proof of credit. In addition to the form, you must complete a course evaluation at the end of each session. If you do not, you cannot recieve credit for the course.
Per ACPE requirements, these programs are valid for three years.
